Information Required:

The following information needs to be completed and tuned into the Nursing Office by August 1st

1. Physical Form- completed by a physician or nurse practitioner.
2. Immunization Form- need to also provide the documentation that proves you have received the
immunizations.
3. Health Form- completed by the student.
4. Background Check- complete the form online, this provides a check for potential felonies
or misdemeanors the student may have. The information is sent to the Director of Nursing Education.
5. Current CPR card.
6. Current Health Insurance card.

All students must maintain current on immunizations, CPR, and health insurance while in the nursing program.

White lab coat (any style)
Royal Blue Scrub Top and White Pants- at least 2 sets
White comfortable shoes- (WHITE ONLY)
Watch with a second hand
Stethoscope
Bandage scissors
Kelley clamps
Goggles
Penlight
Black pen
